1) Telephone: Please call 805-482-3660 and leave a message with (1) your Reservation Name, (2) the Number of Meals, and (3) your Telephone Number (so we can call you to confirm your reservation). Please pay at the BBQ Event by cash or check (make check out to PVHS - please do not mail check).
2) Email: Please email pvhs@pvhsonline.org with (1) your Reservation Name and (2) the Number of Meals. Please pay at the BBQ Event by cash or check (make check out to PVHS - please do not mail check).
3) 'Let's Take Out' - Reservation/Purchase form (below): Please follow the instructions on the form to automatically make your reservation and select your payment method (PayPal/Credit Card, Cash or Check).

First) Please select the number of individuals below. If the number of individuals is greater than twelve, select ‘Number of People @ $15.00/Each’ and enter the corresponding dollar amount. Please also note the number of individuals in the ‘Comment’ field below.
Second) Please select your Payment Method:
> If PayPal (and credit card) payment is selected, please enter your name and email address (and a include a comment if applicable). Please verify your purchase total and click the Submit button. You will be directed to PayPal to complete your payment. Your Reservation and Payment Conformation will be emailed to you.
> If Offline payment is selected, please enter your name and email address (and a comment if applicable). Please bring a check, or cash, to the BBQ Event. Instructions for the check will be present at the bottom of this form. Please verify your purchase total and click the Submit button. Your Reservation Conformation will be emailed to you.
