Pleasant Valley Historical Society Museum Membership
Your Membership donation serves the community by preserving its heritage! Noteworthy, is your Membership includes unlimited access to the Museum, Garden, Gift Shop and Gazebo*. You will also be added to the Newletter Mailing List for the Museum's quarterly newsletter 'The Echo', and to the 'The Echo Updater' for news and event updates as they happen. All contributions to the Museum are tax-deductible as permitted by law.
*The Gazebo can be reserved for small weddings and events.

To use the Membership Form, click on the Membership Form button below; it will download the Pleasant Valley Historical Society Membership Form (.pdf).
Please (1) print*** and (2) complete the form. Please (3) make a check payable to 'Pleasant Valley Historical Society and Museum', and (4) on the memo line of the check please indicate the donation is a 'PVHS Membership'. Please (5) mail your completed form, and check, to:

*** If the Membership Form will not print for you, please don't feel frustrated; simply forget the printing, and follow the steps below.
Please grab a pen and a clean sheet of paper, and jot down the following (for our Accounting Department):
1) Your Name, Address, Telephone Number and Email Address.
2) Please select, and jot down, your choice for your annual contribution ('Individual' through 'Benefactor') or a 'Lifetime' membership:
- Individual -- $25
- Supporter -- $40
- Patron ----- $100
- Corporate - $150
- Benefactor - $250
- Lifetime ---- $500 (One Time Donation)
Annual membership dues ('Individual' through 'Benefactor') are for the calendar year.
3) If you are also contributing the Museum Expansion Project Fund, please jot down 'Expansion Fund' and your donation amount. Thank you.
4) If you are interested in being contacted about the Museum Will Legacy, please jot down 'Museum Will Legacy'.
5) Please make a check, for the sum of your contribution (Membership and Expansion Fund), payable to 'Pleasant Valley Historical Society and Museum'; and, on the memo line of the check please indicate the donation is a 'PVHS Membership'.
6) Please mail your note (for our Accounting Department), and check, to:
